Information you provide
The driver inquiry form collects your name, email address, phone number, home state, CDL-A driving experience, preferred route, confirmation that you hold a Class A CDL and are at least 21, and your consent to recruiting contact. We also record the inquiry time and a reference identifier.
This is an initial inquiry, not a complete DOT employment application. Do not submit Social Security numbers, dates of birth, CDL numbers, license photos, medical records or payment information.
How we use information
We use your inquiry to review your recruiting interests, contact you about driving opportunities, and manage related recruiting follow-up. Submitting an inquiry does not guarantee employment or a particular assignment.
Storage and access
Accepted inquiries are stored in a database on our server. Access is restricted to authorized people supporting recruiting and the operation of this website. Technical service providers may process information to host and maintain the service. We retain inquiries as needed for recruiting follow-up and applicable recordkeeping obligations. Contact us to request deletion or ask about retention.
Technical information and security
The website may process technical request information, including IP addresses, to serve pages and prevent abuse. Abuse prevention uses a derived identifier and time-limited request counts. We do not intentionally include inquiry form contents in routine application logs. No online system can guarantee absolute security.
Cookies, analytics and sharing
This website does not use advertising trackers or analytics cookies. We do not sell driver inquiry information. We may disclose information when required by law or as necessary to protect the service and address abuse.
Your choices
You can choose not to submit an inquiry. To request access, correction or deletion, or to ask us to stop recruiting follow-up, email hiring@hazologisticsllc.com. We may need to verify your identity before acting on a request and may retain information where required by applicable obligations.
